# Add a question to Memorix

A question joins Memorix by itself. MedLibro makes a card when you get the question wrong. There is no button that adds a card by hand.

## When MedLibro makes a card

MedLibro makes a card when you select **Check** and your answer is wrong. This works in these places:

- a session
- a playlist
- a question or a clinical case that you open from a link

An answer that is only partly right counts as wrong here, whatever your scoring rule.

MedLibro makes no card in these cases:

- Your answer is right.
- You select **Skip**.
- You read a session or a playlist in **Explore**.

The new card waits in Memorix at once, so you can review it immediately.

## Clinical cases

Each question of a clinical case stands alone. Only the question that you got wrong becomes a card. The card shows the scenario of the case above the question.

## A question that already has a card

A question has one card at most. If you get the question wrong again outside Memorix, the card starts from the beginning. It then comes back the next day. In a deck, [your rating](/en/memorix/rate-a-card) sets what happens to the card.

## A question outside your plan

A wrong answer on a question outside your plan makes no new card. [Memorix and your plan](/en/memorix/your-plan) gives the details.
